Anémie en pré-dialyse

Retour sur le congrès européen de néphrologie à Munich où j’ai assisté au colloque sponsorisé par l’industrie sur la prise en charge de l’anémie en pré-dialyse.

Where are we now in managing anaemia in patients with ND-CKD?
(voici les présentations des 3 orateurs, à savoir Bernard Canaud; Iain MacDougall et Simon Roger)

Dr Bernard Canaud parle de cela en revoyant les études avec EPO et en s’attardant sur la correction du bilan martial. Voici ses conclusions:

Management of patient with non-dialysis CKD usually consists of nephroprotection

Today, optimal care of patients with non-dialysis CKD should correct anaemia and iron deficiency

Prof Iain MacDougall parle du métabolisme du fer et plus particulièrement de l’hepcidine (j’y reviendrai dans un prochain billet).

Enfin Dr Simon Roger pose de bonnes questions auquelles nous n’avons actuellement par forcément de bonnes réponses!

• Is it the wrong target haemoglobin?
• Is it the wrong target Hb for the wrong patient?
• Is it too much erytrhopoiesis-stimulating agent?
• Should iron be the starting point in anaemia management?
• Should there be a target Hb when considering iron repletion in patient with ND-CKD?
